The Park District of Oak Park falls under the Oak Park Public Health Department not the Cook County’s Public Health Department. We requested the Oak Park Public Health Department to consider the revised Cook County ordinance and we were informed that they would not be considering that option for Oak Park.
Religious exemptions are not included in the Oak Park Public Health Department ordinance.

(Example: John turned 5 on December 15; he has until January 26 until he needs to show his vaccination card. Susie turns 5 on January 17; she has until March 7 until she needs to show her vaccination card.)
